Case of the Month: Gynecomastia with Dr. Polo

Gynecomastia or male breast reduction is one of the more common problems for which men undergo plastic surgery. In 2013, nearly 23,000 of these procedures were performed in the U.S.; making gynecomastia the third most common cosmetic surgery for men, behind only rhinoplasty (nose reshaping) and blepharoplasty (eyelid lift).

Most causes of male breast enlargement are benign. In adolescents, transient breast enlargement is normal; however inunnamed (1) some cases it may persist and remain symptomatic, resulting in significant breast tenderness. In adult males, gynecomastia may similarly be caused by elevated endogenous (internal) hormone levels or by obesity. Additional causes include exogenous (external) sources including prescription medications and illicit drugs, such as anabolic steroids and marijuana.

In a small percentage of cases, male breast enlargement may be a sign of breast cancer. Screening for breast cancer, possibly including mammography, should be performed in any male who develops sudden breast enlargement, particularly when unilateral (one sided), in those of more advanced age and in those with increased risk (i.e. family history).

Once a benign cause is confirmed, the first line of treatment of gynecomastia is to remove any identifiable exogenous source. If none can be identified or if signs and symptoms persist, then surgery may be considered.

Surgical treatment for gynecomastia includes various techniques, alone or in combination, depending on the type. In the simplest type of gynecomastia, there is enlargement of breast tissue immediately beneath the nipple and areola, resulting in a firm, prominent, often tender and painful mass. This condition is most often treated by direct surgical excision through a well disguised incision just at the margin of the areola. In more complex types of gynecomastia, such as in the patient featured in the photos, there may be more diffusely enlarged fatty breast tissue, possibly combined with localized prominence of dense breast tissue beneath the nipple and areola. These cases are sometimes treated with liposuction alone but often also require direct excision of the more firm breast tissue.

In the most complex types of gynecomastia such as that seen in men who have had massive weight loss, the residual breast enlargement is composed primarily of fatty tissue and redundant skin. Surgical correction for these patients involves skin reducing breast lift procedures, often in combination with some amount of liposuction.

When performed by properly qualified and experienced plastic surgeons such as ours here at Miami Plastic Surgery, gynecomastia surgery is associated with a very high degree of patient satisfaction. Patients routinely experience little post-operative pain and discomfort and quickly return to regular activity.

NEW Laser Hair Removal Packages

There are several factors that will dictate how many sessions are required to achieve good results. A few of these factors include skin type, hair color, hair density and hair follicle diameter. Q & A with Dr. Kelly

Q. I keep hearing about “restoring facial volume”. What does that mean? A. Over the past few years, plastic surgeons have unnamed (5)come to realize that the biggest contributor to facial aging is loss of fat. This makes sense to most people because when you look at the elderly, they usually look very hollow. When the facial “balloon” deflates, the facial shape goes from heart shaped to square. We lose the fullness of youth in our cheeks, and jowls develop as the remaining facial structures descend.

This understanding of how facial aging occurs through loss of fat has led plastic surgeons to focus on restoring volume in the face. Temporary fillers such as Juvederm and Restylane are injectable products that can be used to replace fat loss in facial aging and can last up to 6 months.

Voluma is a new product that is used primarily in the upper cheek(or malar) areas and does a great job restoring a youthful look in that area. Voluma , while more expensive than Restylane or Juvederm, looks very natural and can last up to 2 years.

Finally, the best source to restore facial volume is the patient’s own fat. We now harvest fat from the patients’ hips or abdomen to use as fat grafts in the face. This transplanted tissue lives in the new area and is very long lasting and in some cases, may last the patients entire life. While the grafted fat may last a long time, we haven’t stopped facial aging altogether and so the patient will continue to lose their own facial fat as they age. Thus, fat grafting may be need to be repeated down the road to fill other areas of ongoing native fat loss.

Facial volume restoration is a hot topic these days because our understanding of its role in facial aging has allowed us to do a much better job at facial rejuvenation.

Miami Plastic Surgery

Approximately half of all women are affected by the unsightly, and highly annoying spider veins.

Spider veins are dilated or stretched blood vessels that have a branching pattern and are commonly found on the thigh, lower leg and occasionally on the face. Spider veins are progressive in nature and usually increase in number and visibility with age.

Sclerotherapy and other laser vein treatments are used to treat these vessels, erasing them from your skin. Sclerotherapy involves an injection of a solution directly into the vein. The solution injected, Asclera, acts as a sclerosing agent that damages the lining of the treated vessel, leading to a collapse of the vein with eventual absorption by the body. Over time, the vessel turns into scar tissue that fades from view.

A study done by Asclera shows that 86% of patients are happy with their treatment. Unlike hypertonic saline, Asclera is a unnamed (7)much more comfortable solution to inject.

Scleroptherapy is administered by our expertly trained Physician Assistant, Carolyn, who works out of both our Coral Gables and Baptist locations and can accommodate evening and Saturday appointments.

Sclerotherapy consultations are complimentary, and multiple treatments may be recommended for optimal results.

A typical sclerotherapy session lasts 15 to 45 minutes. Post-treatment compression is necessary for optimum results and to reduce the risk of deep vein thrombosis.

Rock N’ Run with Dr. Wolf

The two-part event, which will be held at Temple Beth Am, will have a 5K walk/run, as well as a special VideoSync dance class.

    My name is Bernice, l will like to find out if you can help me solve my skin problems.
    1. l had a motor accident and got a lot of scars on my legs and my hand.
    2. l have wrinkles on my thighs and legs. Am only 32 but look like a 80yrs because of the wrinkles. l don't have it on any where of my body except my thigh up to my leg.. so l can't even wear bikini or anything short. some of my family too has the same problem. Is it a deficiency or what could it be.
    Is there any solution for us ?
