What transpires during a stem cell procedure?

One day I was talking to a patient (Victorio) in the procedure room while extracting his stem cells from bone marrow and fat. This procedure is performed under “conscious sedation,” which means that the patient is awake during the entire procedure (and is under no pain), unlike the case when using general anesthesia during a surgery. During this time, the patient is fully capable to speak with the doctor, as he is guided through the complete procedure. For many people, this is a great advantage to surgery, which many times is rather invasive and the patient is completely unaware of what going on and also possess the risk of anesthesia induced negative side-effects.

We had already successfully implanted Victorio’s stem cells on his right shoulder two years ago and since then he had successfully recovered his mobility almost completely and without the need to undergo a complicated and invasive surgery.

Our conversation switched back and forth between two subjects. First, Victorio wondered why his right shoulder and left knee had given him so much pain, but other joints did not. Then Victorio asked me: “Were they not all supposed to hurt?” Unfortunately, my answer to this common question is still in the hypothesis stage, but I hope to be able to answer it with much more certainty very soon.

It should be noted, that each person has specific genetic information inherited from their parents and is “custom-made” for each individual, sort of like having a “hard drive” (similar to your computer), which is personalized for each human being. In the case of Victorio, his hard disk was “programmed” so that the amount of stem cells in his body “diminished” at 65 years old, particularly on his right shoulder and his left knee.

As we already know, stem cells are responsible for repairing the tissue that wears. They work to repair and rejuvenate the tissue after minor tasks and daily work. To give this action a better perspective, I often use the example of: “opening a brand new hotel and never have it properly maintained, cleaned and/or ever renovated” and thus, after some time all you would find are broken pieces of furniture and dirty / unkept rooms etc.

The conversation then transitioned to the topic of excess synovial fluid in the knee. Many believed that it was rather detrimental to extract it, since that process could invoke the possibility of even more liquid to form. And although the reality is completely different, I think it is important to clarify the definition of this particular fluid:

Synovial fluid is the nourishing medium of cartilage. When it begins to wear out, our organism “innocently” interprets that something is wrong and increases the production of said liquid to “save the cartilage.”

Unfortunately, in wear cases this process does not work that way. If we remove the liquid without repairing the cartilage, the body would still interpret that increasing the production of synovial fluid could feed the cartilage better, which we know is not possible, and therefore, would flood the knee unnecessarily, without providing any improvement.

This is why when we extract the liquid from a knee with cartilage wear, we must also have to implant stem cells, as they proceed to repair and regenerate damaged cartilage and therefore, the body will then stop producing the liquid. Through the use of stem cells, we can fix the “misinterpretation” of the body in order to best heal the body, without the unwanted excess fluid to be formed again.
