Cleveland Clinic Weston Hospital Launches Hospital Care At Home

All Cleveland Clinic hospitals in Florida now offer acute care at home for eligible patients

 

Cleveland Clinic is expanding its successful Hospital Care at HomeSMprogram to Weston and Broward County today. The program launched in April 2023 at Cleveland Clinic Indian River Hospital in Vero Beach and expanded first to Cleveland Clinic Martin Health and now Weston Hospital.

 

Hospital Care At Home allows patients to receive hospital-level acute care in the comfort of their own homes. During its first 18 months, Hospital Care At Home has reduced hospital readmissions and helped more than 1,800 patients recover successfully at home.

 

Patients are constantly connected via technology. Doctors and nurses from Cleveland Clinic efficiently monitor patients and are available at the push of a button. Support staff deliver and install tablets and other technology at the patient’s home the moment the patient returns home, immediately connecting with their care team. Patients with congestive heart failure, pneumonia, kidney infections and other conditions can receive care at home and be with their families, benefitting from the same level of care they would receive in the brick-and-mortar hospital.

 

“When I’m talking with Hospital Care At Home patients virtually, I’m seeing them in their own home, in their own sense of peace,” hospitalist Irene Rahman-Garcia, MD, said. “That’s where their family is – where their dogs and their cats and their loved ones are. It’s important that we acknowledge how significant it is for people to be in their home environment to be able to continue to get better.”

The majority of Hospital Care At Home admissions transfer directly from the emergency department, bypassing the medical wards entirely. Patient experience scores surpass those from the brick-and-mortar hospital.

“Patients report high satisfaction with the in-home technology and the ease of its use, improved communication with their doctors, and an overall higher rating of their hospital care,” Rick Rothman, MD, said. As one of the founders of Hospital Care At Home at Cleveland Clinic, Dr. Rothman points out increased interest in working at Cleveland Clinic’s Hospital Care At Home Clinically Integrated Virtual Command (CIViC) Center as more nursing applicants reimagine what it means to deliver acute “hospital” care without the physical demands of a brick-and-mortar hospital. Dr. Rothman has held leadership positions in hospital operations at Cleveland Clinic in Florida and is currently Vice President and Chief Medical Officer at Cleveland Clinic Indian River Hospital, where the CIViC is located.
