Pain in the shoulder region can be quite troubling and can stem from multiple factors. The shoulder joint is a shallow ball and socket joint with cartilage. When healthy, the joint is capable of an amazing range of motion. These motions are mainly assisted and stabilized by the rotator cuff muscles. These muscles and tendons are on top, in the back and in the front of the joint. The shoulder also contains a bursa, which is a lubricating sac between the rotator cuff tendon and the deltoid muscle. There is also a second cartilage-containing joint in the shoulder called the acromioclavicular (AC) joint. This joint connects the collarbone to the front part of the shoulder blade. The bursa can get swollen and damaged because of “impingement”. Usually, this occurs because the shoulder muscles are not working properly leading to the bursa becoming stuck between tight ligaments or bone.

When the cartilage is injured or damaged through wear and tear, the chemical makeup of the joints becomes more toxic and painful. Part of the rotator cuff passes underneath the AC joint and can be affected by downward-pointing bone spurs. These rotator cuff tendons can also become torn. The important thing to remember is that rotator cuff tears are not always painful. It is also important to understand why the rotator tendon failed. Many times it is because of ligaments that have loosened because of the wear. Simply fixing the tendon tear is not addressing all of the underlying problems.
