The home healthcare industry in the United States is growing by leaps and bounds. According to a recent study conducted by the Associated Press, 88 percent of Americans would rather receive long-term healthcare in their own homes as they age. That fact, coupled with the estimation that by 2034 there will be more adults over the age of 65 than children under the age of 18, which explains why the industry is experiencing an annual growth rate of nearly eight percent.

Miami-based Complete Care Management Services (CCMS) has plotted a course to be the leaders of care at home. CCMS, whose Founder and Chairman Mario Espino brings a data-driven, values-based philosophy to the company, launching the venture in 2019.

“CCMS is dedicated to improving patient outcomes in the acute and post- acute stages of recovery by delivering exceptional personalized in-home healthcare of such high quality that it significantly reduces emergency department visits and hospital readmissions,” explains Espino. “We partner with hospitalists, primary care physicians and other medical experts to expand the services usually not offered in the home healthcare setting. We’re reshaping the way home healthcare is provided; no other company is doing what we’re doing.”

According to the Journal of the American Medical Association, patients receiving home healthcare after a hospital discharge has a 30-day hospital readmission rate of about 22 percent. As opposed to CCMS patients, who are readmitted at the remarkably low rate of less than four percent.

The company’s recipe for success is based on a simple yet profound focus on creating a win-win scenario for what Espino calls the Three P’s – Patients, Physicians and Payers. The patient wins through personalized care and improved outcomes provided by the company’s top-notch primary care doctors, nurses and a host of other healthcare professionals.

Partnering physicians win by expanding their services and actionable patient information that allows them to make better informed decisions while increasing revenue. And payers such as private insurers, Medicare and Medicaid reduce their expenses due to significantly reduced hospital admissions and readmissions.

The CCMS patient-centered model focuses on identifying the underlying causes of each patient’s medical, behavioral and social conditions, then addressing them directly through in-home care, telehealth care, care management, remote monitoring of vital signs and a holistic team-based approach to delivering care.

Espino has long been a believer in the value of doing the research and collecting data, and CCMS was an early user of advanced technology, analytics, logic and artificial intelligence to better provide for patients with chronic diseases and other serious conditions.

“Simply stated, the better you manage the patient, the more control you have over the outcome of the patient’s quality of life,” says Espino. “Our Doctors and nurses deliver compassionate, high-quality care at the patient’s bedside, with a focus on preventive medicine and data-driven care management. Our values-based model aligns perfectly with the latest standards of the Centers for Medicare and Medicaid Services and the Department of Health and Human Services.”
