Your Health Matters : Community Clinical Linkage Program

Live Healthy Miami Gardens (LHMG) in partnership with Trinity Church/Peacemakers Family Service Center work to ensure that residents of Miami Gardens have access to health services through the Community Clinical Linkage Program (CCL). The CCL Program works to improve health through effective partnerships between the health care system, community-based programs, and public health agencies in order to support community access to resources that help prevent, manage or reduce risks of chronic diseases.

Programs and services available through the CCL Program include:
 Preventive and Primary Healthcare Services and Health Screenings
 Nutrition Education and virtual Healthy Cooking Classes
 Diabetes Prevention Program (DPP)
 Access to Physical Activity Programs
 Senior discounts to local gyms
 Care Coordination
 Health Coaching
