CAC-Florida Medical Centers, a subsidiary of health and well-being company Humana Inc., has achieved a three-year recognition by the National Committee for Quality Assurance (NCQA) for their clinicians’ delivery of quality ambulatory care to patients who have diabetes.

To earn NCQA recognition, CAC submitted data documenting its delivery of care to patients with diabetes. CAC has demonstrated high-quality performance in diabetes management measures and is expected to have fewer patients who develop serious complications due to diabetes, such as kidney disease, heart disease, stroke, amputations and blindness.

“Our team and patient-centered approach to diabetes management and care at CAC-Florida Medical Centers encourages and inspires our patients who have diabetes to live healthier lives,” said Dr. Carline Hyppolite-Oyadiran, CAC-Florida Medical Centers regional medical director. “This NCQA diabetes program recognition is a testament to CAC’s physicians and clinicians who tirelessly work to improve the lives of all patients we have the honor to serve.”

In addition to quality care received at CAC, patients are encouraged to participate in group meetings with other patients who have diabetes, certified diabetes professionals and caregivers. The patients motivate and support each other in these sessions while learning from professionals how to best manage their diabetes. CAC also hosts diabetes education classes in English and Spanish as well as diet, nutrition and exercise classes.

“I thank God for the diabetes program at CAC-Florida Medical Centers,” said Jennifer Mayers, CAC-Florida Medical Centers patient. “It has improved my life a lot, coming here and listening to other people… We learn a lot in the center and we learn from each other, too.”

The NCQA’s Diabetes Recognition Program is based on the American Diabetes Association’s guidelines and scientific evidence supporting key measures, such as blood pressure control, foot examination, eye examination and smoking and tobacco use.

CAC-Florida Medical Centers previously achieved consecutive three-year recognitions for excellence in delivery of quality diabetes care in 2009 and 2012.
